General Information
Membership Number: You must be an NAGC member in good standing at the time we receive your convention registration to receive the member discount. There is one discount per membership. Please indicate your membership number in the space provided. If you are not an NAGC member, you will receive a one-year membership with your registration at the nonmember rate.
Badge Name: Use the space provided to indicate how your name should appear on the badge.
Organization: Indicate the school district or organization you represent or by whom you are employed.
Address and email: Please provide a complete mailing address and an accurate email where NAGC can send registration materials.
Please indicate whether this is a home or work address.
Special Considerations: If you require special considerations to attend the convention, check the box and explain; you may attach a letter as necessary.
You may register for the full convention and/or a pre-convention event on Wednesday and/or Thursday. There is no one-day convention registration.
Full-Time Graduate and Undergraduate Students: In order to register as a graduate or undergraduate student, attendee must be enrolled in at least 9 credit hours and must provide copy of his/her class schedule.

Pre-Convention Activities
All pre-convention activities are assigned on a first-come, first-served basis. Pre-convention activities without sufficient registrations may be cancelled in advance. All pre-convention activities include lunch.
Wednesday, November 14, 2012 You may register for either an Action Lab OR Gifted Education Essentials, but not both.
Action Labs
Select your first and second choice on the registration form. You must register for an Action Lab by the Early Bird Registration deadline, September 21, 2012. There are no waiting lists for Action Labs.

Important Deadlines & Onsite Registration
If mailing your registration: Early Bird Registration must be postmarked by September 21, 2012. Advance Registration must be postmarked by November 5, 2012. All registrations postmarked after November 5, 2012, will be treated as Onsite Registrations and will be assessed a $30 onsite surcharge. NAGC does not accept faxed registrations after November 5, 2012. NAGC strongly recommends you register online at www.nagc.org or bring the form with you to Denver.

Methods of Payment
Full payment or a purchase order must accompany each registration. NAGC accepts the following forms of payment: Checks (made payable to National Association for Gifted Children); Credit Cards: Visa, MasterCard, Discover, American Express (Please include expiration date, CID and signature): or Purchase Orders: You must include the original PO with your registration; a PO number only is not sufficient.

Cancellation Policy/Changes
Cancellations must be received at NAGC in writing by mail or fax no later than Monday, November 5, 2012, and will be charged a $100 processing fee. This fee applies to cancellations and changes to your registration. Action Lab fees are non-refundable after September 21, 2012. Refund checks will be mailed after January 1, 2013.
